Our practice accepts insurance for patients. To best serve you, We can verify your insurance benefits before you arrive. Please follow the steps below. Someone will contact you as soon as the benefits are established.
Currently, there are certain plans in NY state that cover Massage Therapy based on necessity as determined by your physician and massage therapist.
These are the verification steps to find out if you are covered, and the requirements thereafter.
I. What you need to prepare:
Insurance Requirements——You will need a prescription which must include:
-Patients name and Date of Birth
-Diagnosis codes or description
-Prescribed Treatment, Medical Massage or Massage Therapy (Not PT or PhysicalTherapy)
-Prescribing physician name, phone #, address, and professional credentials -Date prescription written
-Frequency and Duration of Treatment
*Your physicians office can also fax the prescription to: 631-939-2405 or contact Rachel 516-375-0440 to schedule an appointment with our physicians.

*Most plans have an individual in- network and Out-of-network deductible that will need to be met.
