Currently, there are only a few plans in NY state that cover Massage Therapy based on necessity as determined by your physician and massage therapist.
These are the verification steps to find out if you are covered, and the requirements thereafter. *Most plans have an individual out-of-network deductible that will need to be met.

You will need a prescription which must include:
-Patients name and Date of Birth
-Diagnosis codes or description
-Prescribed Treatment, Medical Massage or Massage Therapy (Not PT or PhysicalTherapy)
-Prescribing physician name, phone #, address, and professional credentials -Date prescription written
-Frequency and Duration of Treatment
